Student Requirements for Athletic Clearance:

1. Download or get a copy of CIF Physicians Statement & Parents Consent. 

2. Have a physical exam and have the doctor complete the consent form. If a player is having problems scheduling an appointment. CVS Minute Clinics (closest one is in Arcadia on Baldwin) offers athletic physicals for $74.   You can schedule a visit online on their website or app.

3. Complete information on Athletic Clearance. 

4. Be sure to upload an image of your medical card and signed physical. 

5. After you are cleared you will receive an email information you that you are cleared. 

This process must be done every year for a student-athlete to remain eligible.
